PDF ebook QA review

Items to check during PDF ebook QA review:

Test some random bookmarks
- If book doesn't contain table of contents page, verify "Beginning of text" bookmark exists at a minimum
Verify page numbering begins on the first page of the main content
Verify front and back cover are the same size as the interior
Verify no gutter overlap (ie, image continuous from left page to right page)
Verify no "artifacts" (eg, hidden layers that become visible when the interior layers are flattened)
Verify no FPO images
