Ingestion failed due to invalid byte sequence

The following ingestion failure can occur when dealing with files originating from a Windows computer:
CSV file not readable: invalid byte sequence in UTF-8
The error is caused by non-standard handling of UTF-8 characters. It is commonly associated with Microsoft Excel.
To resolve, we recommend using Google Docs to create the CSV file.
